Tracking Forms

All team members use these forms to track daily nutrition and physical activity progress toward personal wellness. Print a clean copy every week for individual team members. This form will change with each phase to reflect current goals and midway through each phase to reflect new bonus point opportunities.

Check back periodically; as new bonus points are added, the forms will be added. Dates when new bonus points can be used are available on the program calendar.

Phase 1 (September 18, 2007 – November 12, 2007)

Individual Tracking Form (Phase 1) (New Team Bonus Points Starting October 16 - Remember Team Leader tracks team bonus points)

Sample Individual Tracking Form (Phase 1) — This form shows a sample of how individual team members track weekly progress.

Group Tracking Form (Phase 1) — Team leaders use this tracking form for reporting total team progress every two weeks. Remember to enter the actual dates onto the group tracking form for each two-week period to help you stay organized. Team Leaders: use this form for tracking "team bonus points." (New Team Bonus Points Starting October 16 - Remember to check back for a new Group Tracking Form.)

Newsletters

All team members can download and read these newsletters for specific information related to program goals and strategies. Newsletters will address ways to stay focused on realistic changes and will also provide more information on specific goals than the tips alone can provide. SNA will send all team leaders emails when newsletters become available. Please share the link or print copies for your team members. There will be approximately one newsletter per month.
